Nuclear Operator Admits ''Lack of Safety Culture''
When this tsunami hit the Fukushima nuclear power plant in March 2011 it led to the world's worst nuclear accident in 25 years. On Friday the plant's operator TEPCO admitted a parliamentary inquiry was right when it said the disaster was ''man-made.''
